Kakushu-shomeisho-hakko (The Issuance of Various Certificates)

For the application of the issuance of various certificates such as a Certificate of Completion, Graduation Certificate, etc., please apply using one of the methods outlined below.

Types of Certificates

The Institute of Advanced Media Arts and Sciences
  1. Certificate of Completion
  2. Academic Transcripts (Sealed)
  3. Certificate of Degree Awarded
  4. Certificate of Earned Credits (Sealed)

International Academy of Media Arts and Sciences
  1. Certificate of Graduation
  2. Academic Transcripts (Sealed)
  3. Certificate of Earned Credits (Sealed)
  4. Certificate of Title Conferment (Only for graduates of the Multimedia Studio department)
  5. Certificate of Completion (Research students only)

Handling Fees

300 yen per certificate.

Time Required for Issuance

Since issuance requires about two weeks, please apply well in advance. The issuance of certificates in English requires even more time.

Application Method

As for application methods there are two options available: (1)Online application, (2)Application via post. If possible, please use the “Online application.”

  1. Online application
    It is possible to complete the entire process, from application to payment of the certificate issuance handling fee. Payments can be made by either credit card or PayPay.

  2. Application via post
    After confirming the “Required Documents for Issuance,” please send a genkin-kakitome (registered mail for the purpose of sending money) to the Academic Affairs Department of the Institute of Advanced Media Arts and Sciences.

Required Documents for Issuance

  2. ID Verification Document (Please enclose a copy of any of the below)
    All forms of ID must be in date and valid.
    • My Number Card
      Only the side with a photo has to be attached.
      ※Please do not attach the reverse side which has your personal identity number.
    • Driver’s License
      Both the front and back must be attached.
    • Passport
      All the parts which mention your full name, present address, and birth date must be attached.
    • Residence Card
      Both the front and back must be attached.

    ※ In cases where your name has been revised since completion / graduation.
    In addition to the above, one of the documents below which can be used to confirm your previous and revised name is required. However, submission is unnecessary if the identity verification document displays both the previous and revised names.

    • Official copy of part of family register (issued within the past 6 months)
    • A driver’s license with the change recorded on the reverse side (the front side must also be submitted).
  3. Handling Fees
    300 yen per certificate.
  4. Self-Addressed Stamped Envelope
    Please send a “Self-Addressed Stamped Envelope (with a postage stamp affixed) or “Specified Letter Pack Light Envelope.”
    • Make sure to write the address clearly (the delivery address is to be the applicant’s address).
    • Please be aware that in the event that the return postage is insufficient, we cannot send the certificates until the amount outstanding is received.
    • For individuals who would like a reply via express mail or simplified registered mail, please write on the front of the Self-Addressed Stamped Envelope in red “Express Mail” / “Simplified Registered Mail”.
  5. Postage Fees
    After referring to the Postage Fee Table, please attach a postage stamp to your Self-Addressed Stamped Envelope and send it. In the case of a Letter Pack Light, please send a “Specified Letter Pack Light Envelope”.

    For Reference:
    Certificates → A4 sized (210mm x 297mm)
    Weight of certificates → 10g

  6. Considerations
    • On the envelope addressed to the university, please address it to the Academic Affairs Department and write clearly “Application for the Issuance of Certificates”.
    • When requesting certificates in English, please be sure to append your name in the Roman alphabet in the column with your full name on the Request for Certificate Issuance Form.
